In Date Is Not Anchored: The Fall-Protection Gap That Still Passes the Walk-Through

The harness tag was current. The anchor was rated for the wrong load and the fall-clearance math left the worker hitting the deck before the lanyard locked. Subpart M does not grade on good intentions.
